Provides health care coverage to children from birth up to age 19, pregnant women, and entire family.
Healthy Start
Healthy Start — provides coverage to children from birth through age 18 with family income at or below 200 percent of the Federal Poverty Guideline. Pregnant women with family income up to 150 percent of the Federal Poverty Guideline. Children in families with income between 150 percent and 200 percent of the Federal Poverty Guideline must be uninsured in order to get Healthy Start.

How Do I Apply?
- Contact the Consumer Hotline at 1-800-324-8680 or contact our agency to request an application for Healthy Start / Healthy Families
- Visit www.state.oh.us/odjfs/ohp to download and application
- Complete the application then mail it, fax it or deliver it to county agency
- Provided proof of all income
- No face-to-face interview is required
