Health Insurance In-Home Care
This program helps you stay at home instead of going to live in a nursing home, hospital or institution for persons with mental retardation. There are four Medicaid waiver programs; Ohio Home Care, PASSPORT, Individual Options, and Residential Facility.

How Do I Apply?
- Contact the Consumer Hotline at 1-800-324-8680 or contact our agency to request an application for Medicaid. You must complete a face to face interview.
- Complete the application then mail it, fax it or deliver it to the county agency
- Provided proof of all income and resources
- A worker will contact you to schedule your face to face interview
