Medicare Premium Assistance
This program is for Medicare consumers whose income is too high to qualify for regular Medicaid. This program may pay for some or all of your Medicare expenses including premiums, deductibles or coinsurance. This program is known as Qualified Medicare Beneficiaries (QMB). The Specified Low Income Medicare Beneficiaries (SLMB) will only cover the Medicare Part B premium.

How Do I Apply?
- Contact the Consumer Hotline at 1-800-324-8680 or contact our agency to request an application for Medicaid. You must complete a face to face interview.
- Complete the application then mail it, fax it or deliver it to the county agency
- Provide proof of all income and resources
- A worker will contact you to schedule your face to face interview
