This program provides health care services for Institutional long-term care such as Nursing Homes or an Intermediate Care Facility for the Mentally Retarded.

How Do I Apply?
- Contact the Consumer Hotline at 1-800-324-8680 or contact our agency to request an application for Medicaid. You must complete a face to face interview.
- Informing sessions for Nursing Home applicants are held every Thursday at 10 am
- Complete the application then mail it, fax it or deliver it to the county agency
- Provided proof of all income and resources
- A worker will contact you to schedule your face to face interview
